Please inform us if the directors are the same with the shareholders and full wordings of any abbreviation (short form) is given

Please fill up the bank’s name and the branch address. Please state the condition of the signatory (example both to sign, or either one to sign) and please also state who is the checker (some back call it authorizer) and the maker (authorize)
